What to bring:-
- Bring any necessary referrals, medical records, IEP’s, school documentation, etc. that will be helpful in gaining an understanding of your child’s needs.
- Please complete intake documents sent to you via email (IntakeQ) prior to your appointment if you are able to do so.
- If you cannot complete the online forms and email, or print and bring with you, please call the office for assistance.

- Your child’s evaluation will last between 45 – 60 minutes.
- We prefer to conduct evaluations during our slower times of the day. Please be aware, this may require your child to be taken out of school early for the evaluation appointment. After the initial evaluation, we will do our best to find an ongoing treatment time that accommodates your needs.
- Arrive 5-10 minutes before your scheduled evaluation. Anyone arriving 15 minutes or later to their scheduled appointment may need to be rescheduled for another time. Please call the office at 407-877-0029 of you know you need to reschedule or if you will be late.
- Please ensure your child has been fed, hydrated, and is well rested. This will help in the accuracy of the evaluation.
- Sometimes children perform better without their parent(s)/guardian(s) present during the evaluation, and others, especially younger children, will need a parent/guardian to be available and possibly join their child during the evaluation.
- After your child’s evaluation, the therapist will provide you with a brief summary of their initial impressions. You will receive your child’s completed evaluation report and plan of care no more than 7 business days after the evaluation. Depending on your insurance plan requirements, it may take another 2 weeks to obtain insurance authorization. This authorization will then allow your child to be scheduled for ongoing therapy services. Once insurance authorization has been obtained, your therapist will contact you directly to schedule ongoing treatment visits.
- There may be times where we ask for a Behavior Therapist to accompany your child to their Speech and/or Occupational Therapy appointment. Other professionals that we may ask to connect to as a means to assist with therapeutic outcomes include, but are not limited to: Mental Health Counselor, Psychologist, Psychiatrist, Teacher.
- If your child is deemed to not require our services, we will offer a referral or suggestion to seek out other medical or educational assistance.
